| Project Access is a non-profit, community based human services
agency located in Milwaukee Wisconsin that specializes in
providing support and recovery services for persons with severe
and persistent mental illness.
A dedicated, multi-disciplinary team of nurses, social workers,
and other treatment specialists provide care management,
treatment, and rehabilitation services in the community where
clients live and work. Our efforts focus on helping
individuals achieve their potential for independent living and the
highest possible quality of life.
Project Access offers high quality, community based services
through partnerships with Milwaukee County in three program areas:
Community Support Program - this program is
certified by the State of Wisconsin and partners with the
Milwaukee County Behavioral Health Division to provide intensive
treatment and recovery services for adults living in the
community.
Family Care is a comprehensive, innovative
program administered by the Milwaukee County Department on Aging.
Working with the Department, Project Access is a leader in helping
older adults maintain their independence while receiving need
health and social services.
Targeted Case Management utilizes a
client-centered, strengths based, recovery framework to enhance
client skills and capacity for full participation in community
life. The Behavioral Health Division of Milwaukee County
provides the funding support for this service.
